The history of the creation of the Republican Center for Pediatric Orthopedics
The Republican Center for Pediatric Orthopedics was established in 1992 on the basis of the Republican Clinical Hospital No. 3 in accordance with the order of the Ministry of Health of the Republic of Uzbekistan No. 255 of 1992. The first director of the Republican Center for Pediatric Orthopedics was appointed a talented scientist, Doctor of Medical Sciences, Professor B.M. Mirazimov, who became one of the organizers of orthopedic care in Uzbekistan. There were three departments in the center: 1st department - from 0 to 3 years old, 2nd department - from 3 to 7 years old (up to school age) and 3rd department - from 7 and older (school age) by 210 beds, of which 30 beds are designed for day hospital. The head of the 1st department was a doctor of the highest qualification category Sh.G. Alimova, who headed it until 2017. The employees of the 1st department were mainly engaged in the early detection and treatment of congenital hip dislocation in children under 3 years of age. From 1992 to 1993 R.Kh. Tilavov worked as the head of the II department. The department was engaged in the surgical treatment of congenital hip dislocation. From 1993 to 2002, the third department was headed by F.R. Umarkhodzhaev. The department specialized in the treatment of upper and lower limb pathology of the spine pathology. In addition, there was a department for the early detection and treatment of orthopedic diseases in children, whose staff provided consultations in polyclinics in the Tashkent region. In order to increase the scientific potential of the center, introduce the latest achievements of modern diagnostic and surgical methods of treatment and provide practical assistance, employees of the Department of Congenital Hip Dislocation and Scoliosis in Children of the Scientific Research Institute of Traumatology and Orthopedics of the Ministry of Health of the Republic of Uzbekistan, headed by Doctor of Medicine ... M. Kadyrov. Candidates of medical sciences worked in the department. K.K. Kabilov, Ph.D., Muratov U.Sh., Junior Researcher D.A. Abrorzhadzhaeva, junior researcher S.A. Tursunova, junior researcher Erzhanov Zh.B. They introduced open reduction of congenital hip dislocation with corrective osteotomy of the femur and plastic of the acetabular roof into the practical health care of the Republic. Methods of surgical treatment of congenital hip dislocation have been improved, methods of formation - plastic surgery of the acetabulum - have been introduced, arthrographic methods for examining the hip joint, methods of treating scoliotic disease with the installation of a Harrington distractor have been introduced, and about 100 children with congenital hip dislocation and 30 children with scoliotic disease have been operated on every year. In January 1999, the center moved to a new building at Universitetskaya Street, No. 8, Kibray District, Tashkent Region. From 1999 to 2009, it was led by the most talented organizer Sh.S. Azizov. Under his leadership, a major overhaul of the entire building was carried out with the organization of 4 departments: the 1st department of hip joint pathology; 2nd department of spine and chest pathology; III department of upper and lower limb pathology; and a new department IV-th department - neuro-orthopedics, an operating unit with anesthesiology and resuscitation, an admission department with a consultative clinic, a physiotherapy department, a clinical diagnostic laboratory and an X-ray room. In 2002, resident physician of the Department of Hip Pathology S.A. Tursunova defended her Ph.D. thesis on "Early diagnosis and treatment of congenital hip dislocation in children" in which she noted the role of the sonographic research method as a method for early diagnosis of congenital hip joint pathology in newborns. During these years, the leadership of the republic took a number of measures aimed at increasing the level of specialized assistance in the regions and districts of our country. The staff of the center began to travel annually to provide organizational and methodological work in all specialized institutions in the regions. The main form of work in this capacity was consulting patients, identifying children with severe deformities and congenital anomalies in the development of the musculoskeletal system, joint operations with regional specialists and systematic master classes. The program of the master classes includes consultations of patients and joint operations with specialists in the field, as well as seminars on modern technologies for the diagnosis and treatment of orthopedic diseases in children.
From 2009 to 2017, the duties of the director were performed by Ph.D. A.A. Sadykov. Under his leadership, organizational and methodological work and medical preventive work in the center were continued. A department of general orthopedics was organized, headed by a doctor of the highest qualification category M.Yu. Mukhamedzhanov. During these years, dozens of events were held with specialists in the field, as a result of which widespread early detection and treatment began, as well as the introduction of modern methods of surgical treatment and rehabilitation of children with congenital and acquired diseases of the musculoskeletal system in the postoperative period. H.Sh. Alpysbaev in 2009 defended his Ph.D. thesis "Early diagnosis, prevention and treatment of aseptic necrosis of the femoral head in children" The author proposed a working classification of aseptic necrosis of the femoral head, distinguishing 4 degrees of severity of pathology, 6 stages of the dystrophic process, 4 variants of localization of foci of aseptic necrosis in the head and neck of the femur, subtotal and total forms of aseptic necrosis of the femoral head, which allows differentiating treatment tactics and predicting the duration and outcome of treatment. He developed a scheme for early diagnosis, preventive treatment, which allows to reduce the frequency of severe necrosis by 2 times and to obtain excellent and good outcomes in the main group in 64.4% of cases, satisfactory in 25.4% and timely determine the indications for surgical interventions. In 2017, Ph.D. was appointed director of the center. THEM. Kadyrov, who outlined a number of priority tasks for the center: advanced training of the center's staff in the best orthopedic clinics of the Russian Federation and abroad, re-equipping the center with medical and diagnostic equipment, overhauling the operating unit with an anesthesiology and resuscitation department, which were equipped with the latest MINDREY WATO EX-55 anesthesia devices , modern breathing apparatus, a digital X-ray machine with a C-arm X-ray Machine, SHENDA GJ-3 arthroscopes, cardiac monitors, modern operating tables and lamps, renovation of the entire ground floor of the center, computerization of all departments of the center, established contacts with leading centers, in particular with FGBU Russian Scientific Center "Restorative Traumatology and Orthopedics" named after acad. G.A. Ilizarov, methodological guide of the service for pediatric orthopedics of the Republic. The radiological unit was reconstructed and equipped with modern X-ray diagnostic equipment from Canon (Japan) and MSCT, which was attended by a delegation from the Japanese Embassy in Uzbekistan, headed by the ambassador. As a result, the operating activity sharply increased, which reached 1,500 transactions per year (for comparison, in 2015, just over 700 transactions were performed). Since March 2020, he has been the head of the center, Ph.D. A.B. Tilyakov. In recent years, the center's employees have been conducting research in the field of pathology of the hip joint, spine and chest, neuro-orthopedics, and pathology of the upper and lower extremities. In particular, the following have been developed and implemented: reconstructive and restorative surgeries on the hip joint for multiplanar deformities of the proximal femur after conservative treatment of congenital and pathological dislocation of the hip, spine and chest in congenital and acquired pathology of the spine and chest, hardware-surgical methods for treating deformities of the lower extremities and osteoplastic surgery for congenital and acquired pathologies of tubular bones. The issues of surgical treatment of congenital malformations and acquired deformities of the hand and foot are developing at an accelerated pace. Employees of the Department of Pathology of the Hip Joint, headed by the head of Ph.D. H.Sh. Alpysbaev, for the first time in Uzbekistan since 2018, the surgical correction of spastic subluxation and dislocation of the hip and torsion pathology of the lower extremities in cerebral palsy has been successfully carried out. Since 2019, simultaneous centering of the head of both femurs has been performed with intertrochanteric detorsion-varizing-shortening osteotomy of the femur in case of spastic hip subluxation. Over the past 3 years, more than 100 such surgical interventions have been performed in children with cerebral palsy.
